Alkaptonuria is a rare hereditary disorder resulting from an inability to metabolize homogentisic acid. Clinical and radiographic findings relate to homogentisic aciduria and ochronosis. The latter feature is accompanied by typical radiographic and pathologic findings in both spinal and extraspinal sites. The most characteristic manifestations of ochronosis are widespread disc calcification, with loss of intervertebral disc height, and a distinctive arthropathy of the axial and extra-axial articulations. Careful evaluation of the radiographs usually allows an accurate diagnosis by differentiating this condition from degenerative joint disease and ankylosing spondylitis, the two disorders with which it is most likely to be confused.
